I will need a few details prior to booking an appointment for your animal.

 

Due to legislation I require your vets approval prior to commencing the treatment.

 

Please note that usually this does not require a vet appointment if the vet has seen your dog or horse previously. The form for approval can be taken into (or sent via email) to your vets for their information and signature.
